Who is your Life Care Coordinator?
A Life Care Coordinator (LCC), also known as a Geriatric Care Manager, is a licensed individual trained in social work, gerontology, nursing or other health and human services. LCCs are specially trained to conduct a professional assessment, arrange care services, link to community resources, troubleshoot problems that arise, and assist with a residential placement when the time comes. For the caregivers, LCCs are a source of ongoing support and information about the care of your loved one.
